Condition Explained
Degenerative Disc Disease (DDD) refers to the natural age-related wear and deterioration of the spinal discs. These discs act as shock absorbers between each vertebra and are composed of a tough outer ring (annulus fibrosus) and a soft, gel-like center (nucleus pulposus). As we age, the discs lose hydration and flexibility, becoming thinner and less effective at cushioning the spine. This degeneration can affect any level of the spine but is most common in the cervical (neck) and lumbar (lower back) regions.
Although the name implies a disease, DDD is actually a degenerative condition that affects nearly everyone to some degree as they age. For many people it produces no symptoms; for others, the structural changes lead to significant and chronic pain. Factors such as genetics, obesity, smoking, physically demanding occupations, and prior spinal injuries can accelerate the degenerative process.
As the discs thin, the space between vertebrae narrows. This can cause the vertebral bones to rub against each other, the development of bone spurs (osteophytes), and pressure on nearby nerve roots or the spinal cord itself — each of which contributes to the pain and neurological symptoms associated with DDD.
Common Symptoms
- Chronic low back or neck pain that may worsen with prolonged sitting, bending, or lifting
- Periods of severe pain flare-ups interspersed with lower-level baseline pain
- Radiating pain, numbness, or tingling into the arms (cervical DDD) or legs/buttocks (lumbar DDD)
- Muscle weakness in the arms or legs due to nerve root compression
- Pain that is relieved by walking or changing positions but worsens with sustained postures
- A sensation of instability or weakness in the spine with certain movements
Detection & Diagnosis
Diagnosis begins with a thorough clinical history and physical examination, including neurological testing to assess strength, sensation, and reflexes. Imaging studies are essential to confirm the diagnosis and plan treatment.
MRI (Magnetic Resonance Imaging)
The gold standard for evaluating disc health. MRI reveals disc height loss, hydration changes, annular tears, and any nerve or cord compression in detail.
X-Ray (Radiograph)
Identifies disc space narrowing, bone spur formation, and overall spinal alignment. Dynamic flexion-extension X-rays assess spinal instability.
CT Scan
Provides detailed bony anatomy and is particularly useful when surgical planning requires precise visualization of bone structures.
Discography
A provocative test in which contrast dye is injected into the disc to confirm whether a specific disc is the true source of the patient’s pain.
