patient completes the scheduled medical evaluation and the healthcare provider determines that the patient does not medically qualify for certification, the evaluation fee will be refunded in full.<\/p><p>This refund policy does not guarantee certification.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b3ed702 elementor-widget attend the scheduled appointment.<\/li>\n \t<li>The patient provides false or materially inaccurate information.<\/li>\n \t<li>The patient fails to provide reasonably requested information or documentation needed to complete the evaluation.<\/li>\n \t<li>Certification has already been completed.<\/li>\n \t<li>A government agency later delays, denies, or otherwise does not issue the permit.<\/li>\n<\/ul>\nApproved refunds will generally be returned to the original payment method.\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-e78b3bd
