through telehealth.<\/p><p>Telehealth may include telephone, video, electronic communication, or other appropriate technology depending on the circumstances of the evaluation.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-77d0f00 elementor-widget elementor-widget-heading\" data-id=\"77d0f00\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h5 class=\"elementor-heading-title elementor-size-default\">The patient understands that:<\/h5>\t\t\t\t<\/div>\n\t\t\t\t<div data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<ul style=\"margin-left: 18px;\"><li>Telehealth has limitations compared with an in-person examination.<\/li><li>The provider may determine that telehealth is not sufficient to complete the evaluation.<\/li><li>Additional records or an in-person evaluation may be requested.<\/li><li>Technical difficulties may interfere with the visit.<\/li><li>No diagnosis, certification, permit, or specific outcome is guaranteed.<\/li><\/ul><p>The patient agrees to provide accurate information and to confirm identity and location when requested.<\/p><p>By proceeding with the appointment, the patient acknowledges and consents to the use of telehealth for the
