Telehealth Consent
By scheduling and participating in a HealthNote Network telehealth evaluation, the patient consents to receiving healthcare services through telehealth.
Telehealth may include telephone, video, electronic communication, or other appropriate technology depending on the circumstances of the evaluation.
The patient understands that:
- Telehealth has limitations compared with an in-person examination.
- The provider may determine that telehealth is not sufficient to complete the evaluation.
- Additional records or an in-person evaluation may be requested.
- Technical difficulties may interfere with the visit.
- No diagnosis, certification, permit, or specific outcome is guaranteed.
The patient agrees to provide accurate information and to confirm identity and location when requested.
By proceeding with the appointment, the patient acknowledges and consents to the use of telehealth for the evaluation.
